A 69-year-old man with substance use disorder presented unresponsive with elevated creatine kinase and liver enzymes, positive urine drug screening, and MRI findings of widespread symmetric T2/FLAIR hyperintensity in the subcortical and deep white matter, consistent with a drug-induced leukoencephalopathy.
